How the NCLEX works and how it is scored

The NCLEX is a computer-adaptive test. Each question you answer shapes the next one, and the exam stops once the system is confident whether you are above or below the passing standard. For the RN exam you answer between 85 and 150 questions within five hours.

RN questions
85 to 150
Minimum to maximum
RN time limit
5 hours
Including optional breaks
PN questions
85 to 205
Minimum to maximum
Result
Pass/fail
Pass or fail, not a score

Computer-adaptive testing

  • Questions adapt to you. A correct answer tends to lead to a harder question, and a miss to an easier one, as the system estimates your ability.
  • The length varies. The exam ends when the system is confident of your result, when you hit the maximum, or when time runs out.
  • Some questions are unscored. A number of the items are pretest questions that do not count, and you cannot tell which.
  • Getting hard questions is normal. It means the system thinks you are performing well, so it is not a sign you are failing.

The Next Generation question types

Since April 2023 the exam includes case studies and new item types, such as bow-tie, matrix and trend questions, alongside traditional multiple choice and multiple response. They are built on a clinical judgment model, which tests how you recognize cues, analyze them and decide what to do.

How scoring works

You receive a pass or fail, not a numerical score. The RN passing standard is set at 0.00 logits, a measure of ability on a statistical scale rather than a percentage of questions answered correctly. That means there is no number of questions you can afford to miss, and the length of your exam does not reliably tell you how you did.

Official results come from your state board of nursing, usually after the exam. Pearson VUE also offers a paid unofficial results service in some states; check what your board allows.

Common questions

Does getting 85 questions mean I passed or failed?

No. Because the exam adapts, the length and difficulty tell you little about your result. Many candidates who pass answer more than the minimum.

Will I see my score?

It is a pass-or-fail exam, so there is no numeric score to see. If you fail, you receive feedback on how you did across content areas.
