ABFM certification requirements: residency, license and the seven-year rule
To become ABFM certified you complete an ACGME-accredited family medicine residency, pass the one-day Family Medicine Certification Exam, and hold a full, valid, unrestricted license. You’re ‘board eligible’ for seven years after residency; if that period runs out, you need another year of accredited residency training before you can sit the exam.
From ABFM’s one-day exam page, ABFM’s Board Eligibility Policy as quoted by AAFP and Rosh Review, an ABFM editorial in the Annals of Family Medicine, and an ABFM presentation to program directors. Checked October 12, 2026.
What you need
- Completed residency in an ACGME-accredited family medicine program. You can apply before finishing, but your expected completion date decides which exam you can take.
- A full, valid, unrestricted license. ABFM has said you don’t need it to apply, but you must obtain it within the board eligibility period to be certified.
- Compliance with ABFM’s Guidelines for Professionalism, Licensure, and Personal Conduct.
The seven-year rule
Board eligibility begins the day after you complete residency. ABFM’s current wording, as quoted by Rosh Review, says it expires on December 31 of the seventh calendar year after you finish training. There are no extensions: once it lapses, you must complete a year of additional ACGME-accredited family medicine training (or an ABFM-approved alternative) to regain eligibility.
Certification activities before the exam
AAFP’s resident timeline says that before the exam you must earn certification points through self-assessment and performance improvement activities, by ABFM’s certification-activity deadline (February 26 for spring 2026, September 21 for fall 2026). Many programs use ABFM’s Residency Performance Improvement Program (ResPIP) to complete performance improvement as a group.
Residency length
Family medicine residency is 36 months in an ACGME-accredited program, and study guides citing ABFM note that the last two years must be in the same program. Your expected completion date (June 30 or December 31) determines whether you test in the spring or fall window.
